Local travel between facilities within our network Position Overview We are seeking a personable, professional, and detail-oriented Intake Specialist to support our growing Wellness Program. This role serves as a key point of contact for participants and facility staff, ensuring accurate completion of intake documentation, assessments, and onboarding processes. The ideal candidate is comfortable interacting with diverse populations, possesses strong communication skills, and is willing to travel between multiple locations throughout our local network as needed. Responsibilities
  • Conduct in-person participant intakes for the Wellness Program
  • Complete required intake documentation accurately and efficiently
  • Perform assessments in accordance with program guidelines
  • Explain program services, requirements, and expectations to participants
  • Maintain timely and accurate electronic records
  • Collaborate with facility staff and internal team members to coordinate participant onboarding
  • Travel between assigned facilities and locations as needed
  • Ensure compliance with company policies, privacy standards, and documentation requirements
  • Provide a positive, professional experience for participants and facility partners
Qualifications Required:
  • Active driver's license in good standing
  • Reliable transportation and ability to travel between multiple facilities
  • Strong computer and data-entry skills
  • Excellent verbal and written communication skills
  • Professional, dependable, and organized
Preferred:
  • Experience working in nursing homes, long-term care, assisted living, or healthcare settings
  • GNA (Geriatric Nursing Assistant) or CNA (Certified Nursing Assistant) certification
  • Experience conducting assessments, admissions, intakes, or patient-facing documentation
  • Ability to effectively communicate with individuals from diverse cultural and socioeconomic backgrounds What We're Looking For

00 per hour, with compensation negotiable up to $25.00 per hour based on experience, qualifications, and travel requirements.
