Job Description
GENERAL SUMMARY
To provide direct care services for the care, fellowship, and protection of persons who, because of advanced age or physical or mental infirmity, cannot care for themselves, and to help them reach their maximum potential. An individual designated as a "Floater" does not have a set schedule, accepts assignments as needed within the regional service area, and fills in shift vacancies on short notice.
ESSENTIAL JOB FUNCTIONS
1. Provide direct care as needed. Services may include assistance with grooming, bathing, dressing, meal preparation, feeding, toileting, clothes washing, diaper changes, assisting with ambulation, providing client transportation for shopping/appointments, leisure/community activities, and other similar personal care services at the time and place defined in each assigned client's Individual Service Plan. 2. Provide training in behavioral, social, and independent living skills to the client, as required by the client's Individual Service Plan. 3. Monitor your client(s) during each assigned duty shift, as required by the client's Individual Service Plan. Accurately complete all required documentation daily, including documentation of services provided as events occur, and the status of client(s) to be reported on progress notes/service logs and/or progress reports. Submit your completed documentation to the office by the designated date and time. 4. Follow all applicable Agency and licensing/regulatory requirements for providing client services, including prohibiting workers from working more than 16 hours in a 24-hour period for the Agency or in combination of hours worked with another licensed provider during those 24 hours. The 24-hour cycle starts at the beginning of your first shift that day. 5. Follow all established Agency and programmatic procedures for reporting unusual and/or critical incidents involving clients. Immediately notify your supervisor, or another office staff member, if an unusual event occurs involving your client, such as any suspected abuse, neglect, or exploitation, any significant change in the client's health status or home environment, and any problems encountered in performing your required work activities. 6. When mandated per a specific program/licensing requirement, clock in/out of the designated Electronic Visit Verification (EVV) system for your assigned shift by using a suitable smart device capable of identifying GPS location for clock-in/out. 1. Attend and complete orientation and annual training sessions, as prescribed by licensing and outlined in the Agency training requirements, and any specific training recommended/required to ensure the safety of our clients. 2. Follow emergency medical training while attending to the client. 3. Perform non-complex medical tasks as prescribed in the client's Individual Service Plan only when appropriately trained to do so by medical personnel. Non-complex tasks may include administering medications when delegated by authorized medical personnel who provide training and oversight for the performance of this medical task, as prescribed by licensing, and provide documentation related to a client's Individual Service Plan, such as Medication Administration Records and Seizure Logs, etc. 4. Regular and punctual attendance is required, as well as appropriate demeanor and attire, as per the Agency's Dress Code and Footwear Policies. Adhere to the schedule that is assigned to you and report all absences in a timely manner to your supervisor in accordance with Agency and programmatic requirements. 5. Perform all job duties in accordance with prescribed safe work practices, including the use of closed-toe, non-slip sole shoes, using proper body mechanics and/or lifting devices, and the consistent practice of universal precautions.
OTHER RESPONSIBILITIES
1. Attend and participate in staffing sessions and Individual Service Plan meeting for client(s), when requested. 1. Perform other duties related to the client that are within the guidelines of the Agency, state licensing regulations, and the Individual Service Plan. 1. Maintain positive relationships with the referring agency, clients, and clients' family members. 1. Promote cooperative teamwork with all co-workers and office staff.
KNOWLEDGE, SKILLS, AND ABILITIES
1. Must be at least 18 years-old per licensing requirements. Must have the ability to read and write at a level that ensures understanding of the client's service plan, ability to document services provided, and carry out directions competently as assigned. Verifiable work experience providing support to individuals with disabilities is desired but not required. 2. Must have the qualities of patience, empathy, dependability, and the ability to teach others. Must possess the knowledge, skills, and abilities to adequately provide care and support to individuals with disabilities, the ailing, and/or the elderly. 1. Must have sufficient communication and language skills to perform duties and interact effectively with clients and other staff persons. 2. Must demonstrate the core values of professionalism, including well-groomed appearance and attire, excellent interpersonal skills, friendly and open demeanor, punctuality, organization, and dedication to the job. 3. The ability to bend, stoop, sit, stand, reach, and lift as designated by a client's Individual Service Plan. Possess the physical stamina required to help individuals who need assistance with ambulation, and the emotional strength necessary to work with individuals who need personal care services. 4. When required by a client's Individual Service Plan for transport, staff must have a valid driver's license per Louisiana law and access to a properly registered, insured, and inspected vehicle, in safe operating condition. The vehicle must also be operated at an internal temperature that does not compromise the client's health, safety, or needs.
