In-Home Care Provider

Duties Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by taking vital signs, observing symptoms, and reporting changes to healthcare teams Administer medications accurately following care plans and HIPAA privacy regulations Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking Utilize specialized equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces Support clients with memory care needs through engaging activities and personalized routines for dementia or Alzheimer's care Document all care activities meticulously in compliance with healthcare standards and care plans Collaborate with healthcare professionals, social workers, families, and other caregivers to deliver coordinated support Assist with transportation for medical appointments or community outings when required Qualifications Proven experience in home health, assisted living, nursing homes, or group home settings is highly preferred Knowledge of patient observation techniques, medication administration, and vital signs monitoring Certification in CPR and First Aid is required; additional certifications such as HIPAA training are advantageous Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia or Alzheimer's disease is a plus Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts Strong communication skills to effectively interact with clients, families, and healthcare teams Compassionate attitude with a commitment to providing respectful and dignified care Flexibility to work various shifts including mornings, evenings, weekends or overnight as needed Join us in making a meaningful difference!
