In this vital role, you will provide compassionate assistance with daily activities, health monitoring, and emotional well-being within clients' homes or community settings. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor and record vital signs, medication administration, and overall patient health status in compliance with care plans Support patients with mobility using equipment like Hoyer lifts or assistive devices to ensure safety during transfers Prepare nutritious meals tailored to individual dietary requirements while maintaining proper food safety standards Conduct patient observation to identify changes in condition or behavior, reporting findings promptly to healthcare professionals Maintain a clean and safe environment by performing light housekeeping tasks including cleaning and laundry Follow HIPAA regulations to protect patient confidentiality and ensure compliance with healthcare privacy standards Experience Prior experience in home health care, assisted living, nursing homes, or group homes is highly preferred Demonstrated knowledge of Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or autism Hands-on experience with patient monitoring, medication administration, and vital signs measurement Familiarity with care plan development and implementation for long-term or hospice care clients Certification in CPR and First Aid is required; additional certifications such as HIPAA training, are a plus Experience working with diverse populations including seniors, individuals with disabilities, or those requiring hospice support Ability to lift heavy objects safely and operate mobility aids like Hoyer lifts when necessary Join us in delivering compassionate home care that empowers individuals to live comfortably and confidently.
