In this vital role, you will provide compassionate assistance with daily activities, monitor health and safety, and foster a comfortable, safe environment for clients in their own homes or community settings. Your energy, empathy, and attention to detail will help promote independence and enhance quality of life for those you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs, recognizing changes, and reporting findings accurately Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Support clients with mobility using equipment like Hoyer lifts or assisting with transfers to prevent falls or injuries Maintain a clean, safe environment by performing light housekeeping tasks including cleaning and sanitation Document care activities meticulously, update care plans regularly, and communicate effectively with healthcare teams Experience Prior experience in home health, assisted living, nursing homes, or group home settings preferred Hands-on experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer's), or memory care needs Knowledge of patient observation techniques, vital signs monitoring, and medication administration Familiarity with emergency procedures such as CPR and first aid certification required or obtainable upon hire Experience working within HIPAA guidelines to ensure patient confidentiality and privacy standards are maintained Ability to perform heavy lifting safely when required for patient transfers or mobility assistance Background in long-term care or hospice environments is a plus for providing compassionate end-of-life support Join us to provide exceptional home care that empowers individuals to live safely and comfortably.
