Trillium is seeking a full-time Inclusion Consultant (IC), who is the key provider of quality services to our customers and clients. The IC is responsible for supporting clients to build relationships and engage in their community. Additionally, the IC coordinates with funders, residential supports, and families to ensure success.Responsibility and Duties:
Support individuals with disabilities in identifying personal goals, building independence, and engaging meaningfully in their community. Assist clients in developing life skills, community connections, and participation in activities that align with their interests. Provide onsite support and coaching to clients, including identifying and implementing solutions to barriers related to inclusion, communication, transportation, and performance. Facilitate additional training and support for clients and community partners to promote successful outcomes. Maintain accurate case notes, service plans, and progress documentation using our case management system, which includes AI-integrated documentation support. Manage caseloads, required monthly service hours, and weekly schedules to meet client needs. Collaborate with families, residential providers, funders, employers, and internal team members to coordinate services and support client success.Qualifications and Skills:
Experience in teaching skills to individuals with disabilities- preferred.
Requirements:
Valid Washington State Driver's License, your private vehicle to use for business, and proof of current auto liability insurance Able to successfully pass a criminal background search conducted byWashington State Benefits Package:
