Job Description
Community Alternatives Inc is actively seeking motivated individuals to fill an immediate opening in our Beaver office for the Independent Living Mentor. The Independent Living Mentor will provide a variety of services to individuals in need and identified through Beaver county's Children Youth Services (CYS). Services are community based and within the family's home and individual's school while targeting educational needs to be on par with graduation.
Responsibilities:
The Independent Living Mentor will use a team approach to create a Personalized Education Plan for each youth participant by utilizing Children & Youth Services Independent Living staff, family members, and school staff in creating the plan. The Independent Living Mentor will provide role modeling and emotional support for Independent Living high school student participants by spending time with the youth in their home and school setting as set by the education plan and Children & Youth Services Independent Living staff. The Independent Living Mentor will monitor and assist participants' progress on current academic assignments, graduation requirements, or the participant's senior project and other required projects or assignments based on their grade level. The Independent Living Mentor will assist participants in finding outside resources for current and future academic endeavors, including financial aid and entrance exam preparation courses. The Independent Living Mentor will assist participants in choosing their college, junior college or trade school by setting up and accompanying them on campus and trade school visits. The Independent Living Mentor will prepare participants for the emotional impacts of the transition from high school through discussion, education-related activities, and/or life skill activities. The Independent Living Mentor will collaborate with school, family, and Children & Youth Services Independent Living staff to ensure participants' needs are being met from education, transition, and academic perspectives. The Independent Living Mentor will attend weekly staff meetings with program supervisor and monthly supervision with the program director and participate in individual client file reviews with program supervisor as scheduled. The Independent Living Mentor will attend meetings as required with Children & Youth Services Independent Living staff and communicate with staff regarding their cases.Requirements:
