What you'll be doing Assessing patient suitability for home infusion therapy and designing individualised pharmacy plans of care in collaboration with physicians, nurses, and dietitians. Performing pharmacokinetic dosing, evaluating lab results, and making dosing recommendations to optimise each patient's therapy outcomes. Accurately preparing and dispensing IV medications and related supplies in line with USP <797> standards and all state and federal regulations. Managing patient profiles from initiation through to ongoing therapy updates, ensuring documentation is thorough and timely.
