  • Full-time Job Description BluPeak Health is seeking a compassionate and experienced NP or PA to join our Inpatient Palliative Care team.
In this role, you will provide expert palliative care services to hospitalized patients, focusing on symptom management, emotional support, and improving quality of life. You will work closely with an interdisciplinary team to deliver holistic and patient-centered care. Responsibilities
  • Provide comprehensive palliative care consultations to hospitalized patients with serious, chronic, or terminal illnesses.
  • Manage symptoms such as pain, nausea, fatigue, and breathlessness through pharmacologic and non-pharmacologic interventions.
  • Facilitate patient and family discussions around goals of care, advance directives, and treatment preferences.
  • Collaborate with physicians, social workers, chaplains, and other team members to create individualized care plans.
  • Assist in transitioning patients to hospice care when appropriate, ensuring continuity of care.
  • Educate patients, families, and staff about palliative care principles and practices.
  • Maintain accurate documentation of patient care and outcomes in compliance with hospital policies. Qualifications
  • Current and valid Arizona Nurse Practitioner (NP) license.
  • Certification as an NP in Family, Adult, or Geriatric Medicine (palliative care certification preferred).
  • Minimum of 2 years of experience in palliative care, hospice, or related fields.
  • Strong communication, counseling, and interpersonal skills.
  • Ability to manage complex clinical and emotional situations with empathy and professionalism.
