DUTIES OF POSITION
Responsible for managing the patient intake process. Below are the responsibilities including but not limited to:POSITION RESPONSIBILITIES
Answer phones, receive all referral/intake information. Upload referrals and route them appropriately for admission approval. Verify patient insurance and submit Medicaid NOE when appropriate Verify previous benefit period and make appropriate personnel aware Verify hospice order, send appropriate orders to PCP/Facilities and the like Maintain and update stand-up board and Teams EOD report Order supplies for direct patient care and maintain inventory of such Track admission, discharge, certification and recertification dates on all patients. Send out medical records to PCP and facilities bi-weekly Scan and upload files, referrals, and other documentation to appropriate patient EMR Send out and follow up on CTs and track accordingly Track CTs and provide appropriate updates to status to DOCs and appropriate personnel Contact MD offices and the like to obtain necessary clinical documentation Work with Clinical Team to ensure capture and distribution of necessary documentation Work with Business Development Team to coordinate appropriate aspects of the intake/admissions process Assist with orientation of new employees as needed Order DME Check common working files for Medicare Coverage and verification of benefit period. Send out Discharge Summaries to PCPs for signature Send, request, track internal orders for pharmacy, therapies and other services Perform other duties as directed.JOB CONDITIONS
Position may be stressful at times. It is primarily a desk job which essentially involves sitting, standing, stooping and walking, as well as telephone communication. It requires minimal lifting of office records and printouts.EQUIPMENT OPERATION
The job requires the ability to utilize a PC, calculator, multi- line telephone, and other related office equipment.COMPANY INFORMATION
Has access to all patient medical and financial records.QUALIFICATIONS 1.
High school graduate. 2. Experience in a Home Health Care setting preferred. 3. Must be computer literate and able maintain simple records. 4. Must be able to communicate effectively orally and in writing.Pay:
