Job Description

Job Title:
Integrated Care Nurse PractitionerLocation:

Job Overview:
The Advanced Practice Provider delivers personalized, value-based medical care to patients living with chronic kidney disease and end-stage renal disease. This is a field-based role providing in-home and virtual comprehensive assessments, care coordination, and preventative management. The APP works closely with field nurses, community health workers, and physicians to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life.

This is not an office-based position. The provider must live within 10 miles of Scottsdale, AZ and be comfortable traveling within their assigned territory.
Key Responsibilities:
Conduct comprehensive annual wellness exams in the home and virtual settingsPerform health assessments and develop individualized treatment plansPrescribe medications and order diagnostic testing as appropriateEducate patients and families on disease management and preventive careCoordinate care with primary care providers, specialists, and ancillary servicesComplete documentation accurately and timely, including full history, assessment, and care planIdentify and close gaps in care through preventive monitoring and collaborationParticipate in integrated care team meetingsDeliver evidence-based care that improves quality metrics and patient satisfactionAssist patients with accessing educational and support resources
Required Qualifications:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Associate licenseBoard certification (
NP:
ANCC or AANP;
PA:
NCCPA)Current and unrestricted DEA certificateMinimum of 2 years of direct patient care experienceValid driver's license, reliable transportation, and car insuranceAbility to practice autonomously without direct supervisionProficiency with electronic medical records and point-of-care technologyStrong knowledge of HIPAA, OSHA, and applicable federal and state regulations
Preferred Qualifications:
Managed Care, IPA, or Health Plan experienceExperience conducting annual wellness visits in-home or virtuallyBilingual in Spanish preferredExperience in chronic kidney disease or value-based care environments
