Intellectual & Developmental Disability (IDD) Case Manager The IDD Case Manager coordinates and advocates for services that help individuals with intellectual and developmental disabilities live safely, independently, and successfully within the community.

Key Responsibilities:

Assess client needs and develop individualized service plans (ISPs) Coordinate community resources, benefits, and support services Advocate for client rights, safety, and overall well-being Conduct home visits, monitor progress, and adjust services as needed Collaborate with families, providers, and multidisciplinary teams Maintain accurate documentation and case records

Qualifications:

Bachelor's degree in Human Services, Social Work, Psychology, or related field Experience working with individuals with IDD preferred Strong communication, organization, and problem-solving skills Valid driver's license and ability to travel locally
