Job Description

Join Endeavor Health Services as an Intensive Community Team Counselor, where you will play a vital role in the community, providing compassionate support and guidance to individuals in need! In this position, you will work collaboratively within a dynamic team to deliver personalized care, develop individualized treatment plans, and empower clients to achieve their personal goals. As an Intensive Community Counselor, you will be a key member of a multidisciplinary team, providing intensive support to individuals experiencing behavioral health, and social difficulties. This role focuses on delivering targeted interventions to individuals with complex needs, often involving high-risk cases or individuals in crisis who are experiencing complex situations that impact their ability to receive traditional outpatient care. The counselor works collaboratively with other behavioral health professionals including nursing, psychiatric care providers, peers, case managers, and community organizations to ensure that individuals receive comprehensive care and support.
What You'll Do Individual Assessment & Treatment:
Conduct thorough assessments to evaluate the emotional, psychological, and social needs of the people we serve. Develop individualized treatment plans that address the immediate and long-term goals of individuals, providing intensive therapeutic interventions.
Therapeutic Counseling:
Provide individual, family and group counseling, crisis intervention, and case management services to individuals facing behavioral health issues, trauma, substance use, and other personal struggles. Collaboration with
Team:
Work closely with nursing, psychiatric care providers, peers, case managers, and other team members to create and implement coordinated care plans for individuals. Participate in team meetings and case discussions to ensure a holistic approach to individual care.
Crisis Intervention:
Offer immediate, high-level crisis intervention services to individuals in acute distress or experiencing emergencies, ensuring safety and stabilization.
Advocacy and Resource Linkage:
Help individuals access essential community services, including housing, medical care, education, legal support, and employment. Act as an advocate on behalf of the people we serve to ensure they receive the support they need.
Support System Coordination:
Provide support to families, caregivers, or community members involved in the individual's care. Collaborate with external agencies, schools, or organizations to ensure continuity of care.
Documentation & Reporting:
Maintain comprehensive, confidential records of counseling sessions, progress notes, treatment plans, and case outcomes in compliance with ethical, legal guidelines and regulations.
Professional Development:
Stay informed on current behavioral health practices, evidence-based treatments, and community resources to enhance service delivery and outcomes.
What We're Looking For Education:
Master's degree in Counseling, Social Work, or a related field.
Licensure/Certification:
Active state licensure (e.g., LMSW, LMHC, or equivalent) or working toward. Travel Requirements- Ability to travel to individuals' homes, community locations, or external service providers as necessary to ensure comprehensive care and support for the people we serve. This may involve home visits, attending meetings with external agencies, or providing on-site services in various community settings. A reliable means of transportation and flexibility with travel schedules are essential to meet individuals needs effectively.
Compensation & Benefits:
Salary:
$55,267.65 - 68,776/year depending on education, experience, and credentials.
