Job Description

We are seeking a Patient Care Coordinator/Staffing Coordinator to join the Home Health team in our Mission Hills branch on a temporary basis that could turn into an extended opportunity! In this role, you'll play a vital part in coordinating patient services, supporting clinicians, and ensuring patients receive timely, high-quality care. Pay Range:

  • 35 per/hour
Non-Clinical:
$28
  • 32 per/hour What You'll Do Coordinate and maintain daily schedules for Home Health clinicians and patient visits Manage patient referrals and assign clinicians based on physician orders Process and prepare intake referrals accurately and efficiently Assist with nursing call schedules and staffing coordination Verify Medicare and insurance benefits Communicate with case managers and third-party payors as needed Support interdisciplinary care team meetings Maintain patient confidentiality and uphold HIPAA standards Provide excellent customer service to patients, families, referral sources, and staff Support agency goals, quality improvement initiatives, and daily operations Perform additional duties as assigned What We're Looking For Minimum of 2 years of patient coordination, staffing, scheduling, or healthcare office experience Experience in Home Health, hospice, hospital, or medical office settings preferred Knowledge of Medicare guidelines and Conditions of Participation Strong organizational, multitasking, and communication skills Ability to work independently and collaboratively in a team environment Professional demeanor with strong customer service skills Reliable attendance and flexibility for occasional overtime Physical Requirements Primarily sedentary work with occasional walking, standing, lifting, and stair climbing Ability to communicate effectively in English, both verbally and in writing Valid driver's license, proof of auto insurance, and safe driving record required Why Join Assisted?
