Social Services Director City View Post Acute Salary:
$98,280 annually + comprehensive benefits Position Summary Cypress Ridge Care Center is seeking an experienced and compassionate Social Services Director to lead our social services department and support residents and families in achieving the highest quality of life possible. The ideal candidate is a strong leader with a background in skilled nursing or post-acute care, who is passionate about advocating for residents, ensuring regulatory compliance, and fostering a supportive, resident-centered environment. Key Responsibilities Direct and oversee the Social Services Department in a post-acute/skilled nursing setting Develop, implement, and manage comprehensive social service programs Conduct psychosocial assessments and care planning for residents Support residents and families with emotional, social, and discharge planning needs Act as a resident advocate and ensure residents' rights are upheld Ensure compliance with state and federal regulations (including CMS guidelines) Collaborate with interdisciplinary team members (nursing, therapy, administration) Supervise and mentor social services staff Coordinate discharge planning to ensure smooth transitions of care Maintain accurate documentation and ensure timely reporting Qualifications Bachelor's degree in Social Work (BSW) required; Master's (MSW) preferred Current licensure/certification as required by state regulations Minimum 2-3 years of experience in a healthcare or skilled nursing setting Prior leadership or supervisory experience strongly preferred Strong knowledge of Medicare, Medicaid, and long-term care regulations Excellent communication, leadership, and organizational skills Compassionate, patient-focused approach Compensation & Benefits Competitive salary: $98,280 per year Comprehensive medical, dental, and vision insurance Paid time off (PTO) and holidays 401(k) retirement plan Opportunities for professional growth and advancement Supportive team-oriented work environment Cypress Ridge Care Center is dedicated to delivering exceptional care in a compassionate and supportive environment.
