Job Description

Job Purpose:
The Reintegration Specialist at
LIFESTREAM BEHAVIORAL CENTER
is responsible for facilitating the successful transition of individuals from institutional settings back into the community. This role involves providing comprehensive support and guidance to ensure individuals have access to necessary resources and services, promoting their independence and improving their quality of life.
Key Responsibilities:
Develop individualized reintegration plans in collaboration with clients, addressing their specific needs and goals. Coordinate with healthcare providers, social services, and community organizations to secure necessary resources and services for clients. Conduct regular assessments and follow-ups to monitor client progress and adjust plans as needed. Provide counseling and support to clients and their families to address challenges and promote positive outcomes. Facilitate workshops and training sessions to enhance clients' skills and readiness for community living. Advocate for clients' needs within the community and with service providers to ensure access to appropriate support. Maintain accurate records and documentation of client interactions and progress in compliance with organizational policies. Collaborate with a multidisciplinary team to develop and implement best practices for reintegration services.
Required Education:
Bachelor's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution.
Required Experience:
Minimum of 2 years of experience working in a behavioral health setting. Experience in case management and developing individualized care plans. Familiarity with community resources and support services for individuals transitioning from institutional settings. Experience in collaborating with multidisciplinary teams to support client reintegration.
Required Skills and Abilities:
Strong interpersonal and communication skills to effectively engage with clients, families, and community partners. Ability to assess client needs and develop comprehensive reintegration plans. Proficient in crisis intervention and conflict resolution techniques. Excellent organizational skills and attention to detail for managing multiple cases simultaneously. Ability to work independently and make informed decisions in a fast-paced environment. Knowledge of legal and ethical standards related to behavioral health services. Proficiency in using electronic health records and case management software.
