Documentation Specialist - Physical Therapy & Rehabilitation Location Houston, TX (NASA / Webster Area) Position Type Full-Time, Part-Time, or Remote (Based on Experience) About Us We are a growing outpatient rehabilitation organization providing Physical Therapy, Chiropractic, Vestibular Rehabilitation, Sports Medicine, and Injury Recovery services throughout the Houston area. Our team is committed to delivering exceptional patient care while maintaining accurate, timely, and compliant medical documentation. Position Summary We are seeking a detail-oriented Medical Documentation Specialist to assist our providers with clinical documentation, medical record management, and EMR data entry. The ideal candidate will possess strong medical terminology knowledge, excellent organizational skills, and experience working with healthcare documentation. This role is critical in ensuring that patient records are complete, accurate, and compliant with insurance, legal, and regulatory requirements. Essential Duties and Responsibilities Prepare, review, and finalize medical documentation for Physical Therapy, Chiropractic, and Rehabilitation services. Assist providers with completion of evaluations, progress notes, re-evaluations, discharge summaries, and treatment documentation. Enter clinical information into the Electronic Medical Record (EMR) system. Review records for completeness, consistency, and compliance. Assist with ICD-10 diagnosis coding verification and documentation support. Track unsigned notes and follow up with providers as needed. Coordinate medical records requests from patients, attorneys, insurance companies, and other healthcare providers. Assist with chart audits and quality assurance reviews. Maintain HIPAA compliance and patient confidentiality. Communicate effectively with clinical, administrative, and billing teams. Support documentation required for insurance authorizations and utilization reviews. Perform other administrative and documentation-related duties as assigned. Qualifications Experience in Physical Therapy, Chiropractic, Orthopedic, Rehabilitation, or Medical Office settings. Knowledge of medical terminology, anatomy, and healthcare documentation. Experience using Electronic Medical Record (EMR) systems. Familiarity with ICD-10 coding and rehabilitation documentation preferred. Strong typing and computer skills. Excellent attention to detail and organizational abilities. Ability to manage multiple tasks and meet deadlines. Previous experience as a Medical Scribe, Documentation Specialist, Medical Assistant, Medical Records Coordinator, or Healthcare Administrative Professional preferred.
