Family Partner, Edgewood Family Resource Center Reports To:

1.0 FTE, Non-exempt Mission The Edgewood Center for Children and Families ("Edgewood") is the place to begin for any child or youth to get the mental health care, social services, and academic support they need from highly trained, welcoming professionals. Position Summary The Family Partner provides support to parents/caregivers who are enrolled in Edgewood programs. The Family Partner provides outreach, resource information and brokerage, basic needs support, and follow-up and aftercare to families to meet their non-clinical needs through case management, support groups and workshops, and in-kind donation distribution, including the Edgewood Food Bank. Services are provided in the office, in family homes, in other community locations, and through phone/text/email, or video conferencing. Essential Functions The Family Partner must demonstrate the core competencies of the Protective Factors Framework of the
Strengthening Families Approach:
Support parent/caregiver resilience Help families build strong social connections Help families identify and access concrete support in times of need Provide parenting and child development education Help parents promote the social and emotional competence of children Family Partner Essential functions are: 1. Be an Edgewood Ambassador. Outreach to parents/caregivers and provide them with information and help them connect to services to meet their needs, such as enhanced case management, support groups, food pantries, parent-child interactive groups, evidence-based parenting courses, child development workshops, cultural celebrations, Early Childhood Mental Health Consultation services, including child development assessments, and other Edgewood programs. 2. Be a part of the Edgewood System of Care. Collaborate with all Edgewood Staff referring families to the Edgewood Family Resource Center to coordinate and integrate services to families. Educate all Edgewood teams and programs about services offered through the Edgewood Family Resource Center and ensure welcoming and seamless service provision for referrals. 3. Be a Family Advocate. Provide skilled advocacy and case management supports that includes consistent outreach and engagement to families, complete confidential intake and needs assessments, develop individualized family goals and services plans in partnership with families, provide support in identifying and accessing skills and resources to help families successfully meet goals, and provide follow-up and aftercare to help families continue to access needed services. 4. Be a record keeper. Keep detailed case records in a timely and professional manner and in accordance with program and agency standards and deadlines, such as sign-in sheets, evaluations, enrollment forms, client satisfaction surveys, assessments, service plans, and incident reports. Document and record every service performed, whether it be with an individual, group, team, or organization. Applied Knowledge, Skills and Abilities 1. High School Diploma or General Education Diploma is preferred. 2. One year of community, social, or public health work experience. 3. Spanish-language speaking and writing fluency preferred. 4. Must trustfully work autonomously in the field. 5. Ability to use Microsoft Office programs. Physical, Environmental and Mental Requirements 1. Must be able and willing to efficiently travel throughout the city from one location to another. 2. Must be able and willing to lift 20 pounds. 3. Must be able and willing to work up to 4 evenings and 2 weekends each month if required. Conditions of Employment 1. Background check and clearance. 2. TB clearance. 3. If using agency vehicles, DMV clearance. 4. Meet all agency Covid-19 protocols, which includes vaccination and/or providing a legal exception.
