We focus on empowering current and former foster youth, young adults with complex behavioral health challenges, and under-resourced children at risk of entering the foster care system.

Youth Homes is seeking a dedicated individual to join our Community Based Services (CBS) team as a Family Partner - IHBS . This role is a vital member of both the IHBS team and the Child and Family Team (CFT). Drawing on lived experience in childrearing or supporting youth within behavioral health, child welfare, juvenile justice, or educational support systems, the Family Partner empowers caregivers to fully engage in treatment planning, reinforces IHBS interventions in the home and community, and helps families navigate the multiple care systems that influence treatment goals. The starting pay for this full-time, non-exempt position is $21.29 - $22.35 per hour (DOE). We also offer a bilingual pay differential, if applicable. Key Responsibilities Provide empathetic, nonjudgmental support to caregivers by helping them understand IHBS services and the CFT process, building their confidence as advocates, and assisting them in identifying strengths, natural support, cultural factors, and priorities that guide treatment planning. Serve as an active CFT member by contributing a family-centered perspective to the development and adjustment of individualized care plans, preparing caregivers for CFT meetings, and supporting goal follow-through. Support families in applying IHBS-taught skills by modeling intervention strategies and promoting consistent use of these skills across home, school, and community settings. Assist caregivers in navigating child-serving systems and accessing local resources by helping them understand available support, their rights, and Medi-Cal service components, including SMHS eligibility and IHBS requirements. Build strong relationships with ICC Coordinators, IHBS staff, and county partners, providing culturally responsive insights and serving as a bridge between caregivers and Youth Homes to sustain engagement and reduce barriers. Document all family support activities in accordance with Medi-Cal SMHS requirements, including IHBS-aligned collateral and rehabilitation services. Key Qualifications High school diploma or equivalent. Brings lived experience as a caregiver navigating behavioral health, child welfare, probation, or special education systems, along with strong relationship-building skills to engage diverse families. Understands CFT functioning, family advocacy, and community-based support. Works flexibly across homes, schools, and community settings throughout Contra Costa County. Preferred Qualifications Associate or bachelor's degree in behavioral health. Familiarity with trauma-informed care frameworks and family-centered teaming models. Bilingual/bicultural experience matching Contra Costa County's diverse communities. Other Requirements At least 21 years of age. Must successfully complete criminal background check, health screening and other state/federal licensing/certification requirements. Must have reliable transportation and valid personal auto insurance to regularly transport clients, participate in community-based activities, and visit Youth Homes offices throughout Contra Costa County.
