Responsibilities will include but are not limited to the following: Assist the individual/ family in completing the startup process according to OPWDD requirements and program procedure. Assist the individual and family in maintaining the self-direction plan and budget, including monitoring expenditures to ensure that expenditures do not exceed the self-direction budget. Assist the individual to build their Circle of Support and facilitate Circle of Support meetings. Assist the individual in developing a comprehensive Person-Centered Plan with the involvement of the Care Manager and Circle of Support Ensure completion and regular updating of habilitation plans for self-hired Community Habilitation and Supported Employment (SEMP) staff if these services are chosen. Assist the individual to properly document self-directed services according to Medicaid and OPWDD regulations and policy including mileage and expense reimbursement forms, and all other required documentation. Assist the individual's self-hired staff to properly document self-direction services according to Medicaid and OPWDD regulations and policy including timesheets, service documentation, mileage and expense reimbursement forms and any other required documentation. Track the Self-Direction budget through the approval process and act as liaison with the local OPWDD office throughout the duration of application, approval, and implementation, responding to requests for information in a timely manner. Assist individuals as specified by each broker contract of services with such things as annual meetings, plan management, revisions, amendments as well as provide support around employee issues. Ensure that Circle of Support meetings occur at least two times per year and meet informally with the family at least quarterly to review the budget and services Attend and participate in individual conferences, staff meetings, training sessions and professional development opportunities as required. Complete annual professional development and maintain current Support Broker certification as per OPWDD regulations. Maintain necessary records and documentation for each consumer served. Act in a manner consistent with Central New York Quest's Mission. Other duties as assigned by the supervisor.
Salary:
$37,000.00 - $41,000.00 per year

We are currently looking for a full-time Support Broker to work with persons who have elected to self-direct their services. Prior experience as a support broker is preferred and candidates must have OPWDD Broker Authorization. Successful candidates must have an Associate degree in accounting or a related field. A valid driver's license is also required as travel to participant homes and meetings is required.
