Job Description

Summary This position ensures all case files are maintained in accordance with agency and state standards. This position is responsible for the supervision of youth clients placed on case management and/or probation. This position will assist clients in dealing with problematic situations to ensure client continued success and to ensure public safety. The position assists clients in meeting the conditions set by the court so they can successfully terminate probation. Work is performed independently under the general supervision of the CCMA Supervisor and/or his/her designee. Definition and Examples of Work 50% Perform case management services for clients Provide crisis intervention and mediation services or juveniles and families; services may include scheduling and travel. Participate in meetings with clients, school personnel, employers, law enforcement, and other service providers, shop for items for client as needed, and conduct school visits and visits to clients in placement and correctional facilities. Assist juveniles with finance management, employment services, location of housing, and building/maintaining relationships. Write court reports and letters and attend all scheduled court hearing for juveniles, ISP clients, and possible future clients. Provide testimony before the court as necessary. Conduct surveillance as needed and serve as electronic monitoring specialist. Coordinate services with contractual service providers as needed and serve as liaison to the Department of Children and Families. Prepare a monthly client/case report and meet weekly with supervisor and other case managers to discuss clients/cases. Interview, screen and oversee assessment of youth offenders referred to case management or probation. 50% Performs onboarding procedures, assessment processes, and administrative duties Input all client information into Chronos and develops and maintains current and accurate record and database system on youth offenders according to standards established by the 25th Judicial District Youth Services and/or the KDOC. Perform a Youth Level of Service Assessment for all new clients and determine youth eligibility for services and correct placement level on program Complete orientation and termination paperwork and processes for various services/programs including performing drug/alcohol screens on offenders for evidence of substance abuse and probation activation paperwork. Additional duties may be assigned as business needs require. Excellent communication skills, oral and written. Ability to communicate information concisely and accurately so others can comprehend the information being conveyed. Ability to write clear and concise reports and chronological records. Ability to establish and maintain effective working relationships with local officials, employees, outside agencies and people from a variety of socio-economic backgrounds. Proficient in Microsoft Office Suite or related software. Ability to work independently. Ability to maintain confidentiality of information. Ability to access, input and retrieve information from a computer. Ability to accurately interpret laws policies, and regulations Knowledge of all processes of the judicial system Youthful Level of Service/Case Management Inventory (YLS/CMI) certification upon hire Valid Driver's License Occasional Travel Subject to on-call duty Ability to pass a background check Regular and reliable attendance onsite at work location
