Job Description
Position Summary:
Responsible for individualized client services provided in a community/residential setting. Duties include assessing, planning, implementing, monitoring, and evaluating client behaviors and outcomes while ensuring appropriate levels/type of services are provided.Position Description:
Withclosesupervision; conducts interviews andadministers assessments and screening instrumentsMaintain caseloadand productivityby ensuring regular contact with clients tomonitorand assess progressWrite service plans, reviewprogressanddetermineupdates needed in collaboration with clientUtilize core correctional practiceswhileassistingclients in effective community re-integrationDeliveron-going services to clients which mayincludecrisis intervention, advocacy, medication monitoring, transportationassistance, financial budgeting,housingassistance,and referral and linkage to community resources as neededEnsurecompliance with applicable standards and agency policiesDocument all clinical activities to meet specific program,funderand regulatory criteriatimelyand with accuracyParticipate inteammeetingsandassistin making recommendations for client status changesLead educational groupsand/orfacilitategroup curriculumMay require transportation of clients Adhere to professional standards as outlined by protocols,rulesand regulationsOther duties as assignedRequired Knowledge, Skills and Abilities:
Ability to learn case management principles and core correctional practicesExcellent organizational and time management skillsEffective verbal and written communication skillsDemonstrate strong ethical and professional behavior. Bachelor's degree in behavioral science, Criminal Justice or related fieldorequivalentwork experience as allowed/required by regulatory bodiesMay require CDCAor other specialtycertificationEssential Functions/Physical Demands:
