Responsibilities The Legacy Medication Safety Officer is responsible for the coordination of the activities involving medication safety in the organization. This position provides leadership for medication safety assessments, educates providers and staff on the system-based causes for medication errors, communicates best practices regarding effective medication safety strategies to others within the system and interfaces with hospital site and system pharmacy and quality representatives on medication safety issues. The Medication Safety Officer is responsible for review, analysis and trending of all medication events for the system and participation in any root cause analysis involving a medication event as well as creation or review/revision of patient care policies that impact the safe use of medications. The role of this position is to elevate the importance of medication safety in the Legacy Health.
Qualifications Education:
Graduate of an accredited school of pharmacy. Advanced degree (MS or PharmD) and/or residency preferred.
Experience:
A minimum of five years clinical practice experience in a hospital or health system pharmacy practice (or four years plus a year of residency training) required. Keyboard skills and ability to navigate electronic systems applicable to job functions. . Equal employment opportunity, including veterans and individuals with disabilities.
