UChicago Medicine AdventHealth GlenOaks Hospital, 701Winthrop Ave, Glendale Heights, IL Schedule:
Full-time, Monday-Friday 8a-4:30p; Possible remote one day per week Promotes an organization-wide approach to increasing patient safety and reducing errors within hospital settings. Monitors and tracks the safety event reporting system and maintains confidential documentation on investigations. Leads, supports, and facilitates the use of tools and implementation of systems/processes that identify patient safety risks, promote a culture of safety, reduce patient harm, and improve high reliability healthcare. Facilitates and participates in causal analyses, develops RCA and ACA action plans in collaboration with clinical and operational leadership, and monitors and supports timely and effective implementation. Facilitates meetings, initiates task forces or work groups to directly remediate and resolve quality of care, patient safety, or clinical systems issues. Facilitates weekly Safety Event Review Team (SERT) meetings. Analyzes patient care information from multiple data sources and reports on results. Coordinates serious reportable events and sentinel event communication and follow-up with appropriate internal stakeholders and external agencies. Utilizes alerts and best practices to perform gap assessments and implement strong actions that will alleviate identified gaps. Collaborates and oversees the hospital-based Patient Safety Coach program, recruits and trains department patient safety coaches, and coordinates monthly safety coach meetings. Shares patient safety lessons learned and safety alerts across the region and coordinates collaborative patient safety efforts. Assists in the development and delivery of patient safety, high reliability, and culture of safety education, and mentors, trains, and orients staff in patient safety. Oversees and maintains applicable patient safety dashboards, develops and tracks patient safety-related goals, and disseminates information to relevant stakeholders. Maintains collaborative relationships with other teams/departments including Risk Management, Quality and Patient Safety, Accreditation and Compliance, Medical Staff, and Nursing. Participates in the development, review, and revision of hospital policies and procedures that affect patient safety. Participates in relevant committee meetings to offer support and guidance and provides constructive feedback as needed. Rounds in patient care areas to promote event reporting and facilitate communication of "lessons learned" from adverse patient care event investigations and analysis. - Ability to work with an electronic health record. [Required]
- Requires strong interpersonal and communication skills, both oral and written. [Required]
- Ability to work well with senior management, directors, medical staff, and employees at all levels of the organization and externally. [Required]
- Requires excellent computer skills, including advanced use of Microsoft Word, Excel, and PowerPoint. [Required]
- Working knowledge of patient safety science and high reliability principles.
Education:
- Bachelor's [Required]
- Master's [Preferred]
Field of Study:
- from an accredited school/college of nursing, allied health, or healthcare related field.
- in healthcare-related field
Work Experience:
- 5+ experience in hospital/acute care (2 years must be within the past 3 years) [Required]
- Experience facilitating root cause analysis, failure mode effect analysis, and error investigations. [Preferred]
- Experience in quality, patient safety or risk management [Preferred]
Licenses and Certifications:
- Certified Professional in Patient Safety (CPPS) [Preferred]
