Overview At AccuHealth, we believe patients thrive when they feel safe and supported in their own environment. We're seeking a COTA who enjoys partnering with OTs to provide practical, functional care in the home setting. What You'll Do Your responsibilities include providing in‑home therapeutic treatments, functional exercises, and patient/caregiver instruction according to the OT's Plan of Care. You'll document each visit, support goal achievement, and coordinate with a collaborative clinical team committed to safe, effective care. Why You'll Love Working With Us A supportive, approachable clinical team Manageable caseloads and realistic expectations Opportunities to grow your skills in both home health and (optional) hospice comfort‑focused care A culture that values kindness, teamwork, and professional development What You'll Do Provide individualized occupational therapy interventions in the patient's home under the direction and established Plan of Care of the OT Teach patients and caregivers practical strategies for safety, independence, and daily living Document visits, progress, and recommendations accurately and on time Communicate changes in condition to the OT and nursing team Participate in interdisciplinary collaboration, case conferences, and quality improvement activities Ensure safe use of equipment and maintain a safe environment during all visits Follow all agency, state, and federal guidelines, including infection control and confidentiality standards Attend required trainings and maintain professional development Support occasional hospice patients with comfort‑focused, non‑restorative interventions (optional/limited)
Pay:
$48.00 - $55.00 per hour
