This position offers the opportunity to work closely with residents, families, nursing, rehabilitation, and facility leadership to help ensure successful care transitions and positive resident outcomes.

Compensation Licensed MSW:

$83,000 - $93,000/year

Licensed BSW:

$65,000 - $75,000/year Responsibilities Coordinate admissions and discharge planning processes Complete psychosocial assessments including BIMS and PHQ-9 Support residents and families throughout their stay Participate in interdisciplinary care planning meetings Collaborate with nursing, therapy, and administration teams Maintain documentation and compliance within PointClickCare Assist with regulatory readiness and resident advocacy Qualifications Active social work license required ( LCSW, LICSW, or LSW ) MSW or BSW required Skilled nursing, rehabilitation, or long-term care experience required Knowledge of Medicaid and discharge planning preferred Strong communication and organizational skills Position Details Full-time, direct hire Medical, dental, vision, and 401(k) benefits available
