Job Description

Job Summary We are seeking a highly experienced Hospice Registered Nurse (RN) to join our compassionate care team. The ideal candidate must have extensive hospice medicine experience, demonstrating proficiency in managing end-of-life care with sensitivity and professionalism. This role involves providing comprehensive patient assessments, symptom management, and coordinating care plans to improve quality of life for patients in hospice settings. Candidates should possess strong clinical skills, familiarity with EMR/EHR systems(Kinser), and a deep understanding of hospice protocols and regulations. The RN will work closely with patients, families, and interdisciplinary teams to deliver personalized, respectful care that aligns with patients' wishes and needs and possibly transition from Part-Time into Full-Time role. Duties Conduct thorough patient assessments including vital signs, physical examinations, and symptom evaluations. Develop and implement individualized care plans focusing on pain management, symptom control, and emotional support. Administer medications and treatments such as tube feeding, and wound care following aseptic techniques. Manage complex cases involving acute pain management, end-of-life care, and advanced symptom control strategies. Coordinate with physicians, Director of Nurses, social workers, chaplains, and other healthcare professionals to ensure holistic patient care. Document all patient interactions accurately within EMR/EHR system Kinser while maintaining HIPAA compliance. Provide education to patients and families regarding disease processes, medication administration, and advanced directives. Assist with discharge planning and coordinate community resources for ongoing support post-hospice care. Participate in interdisciplinary team meetings to review patient progress and adjust care plans accordingly. Ensure infection control practices are strictly followed in accordance with healthcare standards. Requirements Valid RN license with current registration in the state of practice. Extensive experience in hospice & palliative medicine is a MUST! Candidates must demonstrate proficiency in managing end-of-life symptoms effectively. Strong clinical skills including medication administration (IV infusion, injections), wound care (including ostomy), tube feeding management, and ventilator support if applicable. Familiarity with EMR/EHR systems such as Epic or Cerner is required. Knowledge of medical terminology, anatomy & physiology, ICD-10 coding, and medical documentation standards. Experience working with diverse patient populations including geriatrics, patients with developmental disabilities or dementia care needs is preferred. Ability to work independently while collaborating within an interdisciplinary team environment. Excellent communication skills with compassionate patient-centered approach. Must pass background checks screening prior to employment.
