Responsibilities include but are not limited to: Under the supervision of a Registered Nurse andor licensed Therapist, receives daily patient assignments and carries out assignments in accordance with individualized patient plan of care. Collects patient and family data and reports findings to RN andor licensed Therapist in a timely manner. Assists in the communication of information to assigned patients. Under the supervision of an RN andor licensed Therapist, performs interventions and procedures as directed to ensure the safe effective quality of care for patients. Reports to RN andor licensed Therapist on a variety of aspects of patient and family information including. Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patientfamily's needs. Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulationexercises, etc. Serves as Hospice Aide as directed. Serves as Personal Care Attendant as directed. Proactively reports changes in skin conditions to RN, Case Manager or program manager in a timely manner. and
