In this vital role, you will provide compassionate, skilled nursing services across diverse settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual needs. Administer medications and injections accurately following established protocols, including IV infusions and medication management using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing while maintaining aseptic techniques. Support patients with specialized needs such as dementia care, Alzheimer's care, memory care, behavioral health management, and working with individuals with developmental disabilities. Conduct diagnostic evaluations including specimen collection/processing, phlebotomy, and medical imaging support under physician supervision. Manage airway and ventilator support for critical care patients in ICU or Level I Trauma Center settings; provide emergency response including airway management and acute pain management. Document all patient interactions thoroughly in compliance with HIPAA regulations; ensure accurate coding using ICD-9/10 and CPT coding standards for billing and medical records. Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care to optimize patient outcomes across inpatient and outpatient settings. Provide health coaching on topics such as nutrition, medication adherence, family planning, and home & community care practices. Experience Proven experience as a Licensed Practical Nurse in various healthcare environments such as hospitals (including ICU or Level I Trauma Center), nursing homes, assisted living facilities, or outpatient clinics. Critical care experience with familiarity in acute pain management, infectious disease care, trauma response, and emergency medicine procedures. Hands-on experience with EMR/EHR systems like Cerner or Athenahealth; proficient in medical documentation and ICD coding (ICD-9/10). Skilled in performing venipuncture, IV insertion/infusion therapy, catheter care, sterile techniques, and specimen collection/processing. Experience working with diverse patient populations including pediatrics (neonatal ventilator), geriatrics (memory/dementia care), behavioral health patients or individuals with disabilities. Knowledge of anatomy and physiology principles; capable of conducting physical examinations and diagnostic evaluations accurately. Familiarity with specialty areas such as hospice care, home & community care experience, occupational health services or post-acute sub-acute care settings is preferred.
