Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will help ensure patients receive personalized, efficient, and safe nursing services. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications and injections accurately following prescribed protocols while adhering to HIPAA regulations for patient privacy. Implement specialized care procedures such as IV infusion, catheterization, tube feeding, wound suturing, and sterile processing techniques. Assist with diagnostic evaluations including specimen collection, laboratory processing, and medical imaging support. Manage emergency situations by airway management, ventilator operation, acute pain management, and responding swiftly to urgent patient needs. Document all patient interactions thoroughly using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems like Cerner or Athenahealth. Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes. Experience Valid licensure as a Licensed Practical Nurse (LPN) with current state certification required. Proven experience working in diverse healthcare environments such as hospitals (including ICU and Level I Trauma Center), nursing homes, hospice care, or outpatient clinics. Demonstrated proficiency in medical terminology, ICD-9/ICD-10 coding, CPT coding, and medical documentation practices. Critical care experience including ICU or emergency room settings is highly desirable. Familiarity with EMR/EHR systems such as Cerner or Athenahealth for accurate record-keeping and documentation. Experience working with individuals with developmental disabilities or dementia (Alzheimer's/memory care) preferred. Knowledge of infection control protocols and aseptic techniques essential for maintaining patient safety. Ability to perform complex procedures like spinal taps, cardiac catheterization support, or phlebotomy with precision.
