Licensed Practical Nurse

Job Summary We are looking for a Licensed Practical Nurse (LPN) to provide direct patient care under the direction of a licensed provider (Physician, Physician Assistant, or Nurse Practitioner). This role supports clinical operations by assisting with patient care, procedures, and administrative tasks while ensuring high-quality, compassionate care in accordance with Oregon state regulations and organizational standards. This role will be stationed at our Sutherlin Oregon clinic. Duties Provide direct patient care within the scope of LPN licensure and under the direction of a licensed provider Assist providers with examinations, treatments, and procedures Record patient medical histories, vital signs, and clinical information accurately in the electronic health record (EHR) Administer medications and treatments as ordered by a licensed provider and within scope of practice Monitor patient condition and report changes to the provider Prepare and maintain exam rooms, ensuring proper cleanliness and stocking of supplies Perform clinical procedures such as injections, wound care, EKGs, and specimen collection Support care coordination, including patient follow-up and communication Educate patients and families on treatment plans, medications, and care instructions Maintain compliance with infection control, safety, and HIPAA standards Collaborate effectively with providers and clinical staff Requirements Current Licensed Practical Nurse (LPN) license in the State of Oregon Graduate of an accredited practical nursing program Current BLS/CPR certification Minimum of 1 year of clinical experience preferred (outpatient or specialty setting a plus) Strong clinical, organizational, and communication skills
