Job Description

Schedule:
12 or 8 hour shifts available, days or nights, weekends.PT, PRN, FT
availableResponsibilities/Qualifications:
Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required On each assignment, provides skilled nursing care/services in accordance with prescribed orders Identifies and documents changes in patient/client physiological and psycho-social condition Notifies Director/Supervisor, and contracted customer designee if applicable, when: Physician notification is required in order to obtain necessary orders for intervention(s), per state regulations Changes in patient/client condition have occurred Adheres to Maxim policy, procedure and applicable protocols Identifies and prioritizes actions based upon patient/client care requirements and/or contracted customer requirements Ensures that documentation reflects patient/client collaboration including participation in plan of care creation and maintenance, physician correspondence/collaboration, expected patient/client outcomes, nursing interventions employed, and other items as required by contracted customer Performs/provides interventions, treatments, and medication administration, according to identified priorities, the plan of care, facility policies and patient/client care outcome standards as applicable Monitors and documents effectiveness of interventions, treatments, and medications Assures patient/client safety at all times Monitors the safety and effectiveness of the care/services environment and equipment used, including universal precautions Executes consistent, effective interpersonal communications and maintains positive, professional relationships with patient/client and contracted customer stakeholders Adheres to the following professional practice standards: Nurse Practice Act Standards of Nursing Practice Standards of Patient Care Standards of Accrediting Body State and Local Laws Patient Bill of Rights Policies and procedures of contracted customers Demonstrates adaptability and flexibility to changes in the work environment. Any other duties as instructed by management. Acquire and maintain all CEU, CPR, trainings, etc. as required by DMHA and JCAHO.Requirements:
A minimum of 1 year of experience in related field. Valid LPN License in State of New York/compact license. Knowledge of chemical dependency and substance dependency population. Basic knowledge of detoxification assessment and treatment procedures. Ability to understand and oversee medication management. Ability to react appropriately to interventions common to chemically dependency patients.Pay:
$32.00 - $35.00 per hourLicense/Certification:
