Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions and tube feedings, following strict protocols and HIPAA regulations. Assist with complex procedures such as catheterization, wound suturing, and sterile processing to ensure optimal patient outcomes. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns promptly for triage or emergency intervention. Collaborate with multidisciplinary teams in diverse settings such as long-term care, assisted living, hospitals (including ICU and trauma centers), hospice, and outpatient clinics to coordinate comprehensive care plans. Maintain meticulous medical records, ensuring compliance with

ICD-9/ICD-10

coding standards for accurate documentation and billing purposes. Educate patients and their families on health management strategies, including dementia care, Alzheimer's support, nutrition, and discharge planning to promote recovery and independence. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in various healthcare environments such as medical-surgical units, emergency rooms, ICU, pediatrics (including neonatal ventilator care), or home & community care settings. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly desirable. Strong knowledge of anatomy, physiology, pharmacology, and medical terminology to perform accurate patient assessments and medication administration. Experience with specialized procedures such as phlebotomy, venipuncture, sterile technique, airway management, and IV insertion. Ability to work effectively with diverse populations including individuals with developmental disabilities or dementia-related conditions like Alzheimer's disease. Excellent communication skills coupled with critical thinking abilities to provide exceptional patient-centered care in fast-paced environments such as urgent care or trauma centers.
