Travel:

$18/hr taxable + $840/week Facility Overview A skilled nursing facility with around 142 beds and an average census of about 117-123 residents. It provides long-term care, short-term rehabilitation, and memory care services. Key Facility Notes Medium to large SNF facility Mixed population: long-term care, rehab, memory care Semi-rural location in Athol, MA Uber/Lyft available but limited (longer wait times expected) Cellular signals generally stable but may vary indoors

Responsibilities:

Assist in administer medications and treatments as ordered Provide direct nursing care to residents Monitor patient conditions and report changes Assist with care planning and documentation for the whole medical team Collaborates with RNs, CNAs and support daily patient care activities Take vital signs if needed and maintain accurate records Ensure patient safety and compliance with care standards

Requirements:

Step #1 Updated resume Minimum 1 year of relevant experience LVN Nursing license Current

• BLS/CPR Skills checklist 1 to 2
• current
• Supervisor references Copy or driver's license and SS card COVID Vaccination Card Step #2 Physical (within the last 12 months) PPD (2-step) OR QuantiFERON within last 12 months PPD 2-step acceptable if done correctly Second PPD must be within 3-7 days after the first result

Chest X-ray:
