Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to patients across various settings, including long-term care, assisted living, hospitals, and community health environments. Your expertise will support patient recovery, comfort, and safety through comprehensive assessments, medication administration, and patient education. The ideal candidate will possess a strong foundation in diverse clinical practices and demonstrate a commitment to delivering exceptional patient-centered care. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately while adhering to protocols for IV infusion, catheterization, and medication management. Assist with complex procedures such as wound suturing, tube feeding, and ventilator management in accordance with established aseptic techniques. Monitor patients' conditions closely in acute care environments like ICU or Level I Trauma Centers, providing critical support during emergencies or trauma situations. Conduct patient triage in urgent care or emergency room settings to determine priority levels and facilitate timely interventions. Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with diverse needs including dementia, Alzheimer's disease, developmental disabilities, or infectious diseases. Document all patient interactions comprehensively while maintaining compliance with HIPAA regulations and medical coding standards such as ICD-10 and CPT coding. Support discharge planning by educating patients and families on medication regimens, home care instructions, and follow-up appointments. Participate in case management activities focusing on chronic conditions such as geriatrics or pain management to optimize health outcomes. Engage in ongoing professional development to stay current with medical terminology, physiology knowledge, and evolving healthcare practices. Qualifications Valid Licensed Practical Nurse (LPN) license from an accredited nursing program. Proven experience working in diverse clinical settings including hospital medicine, long-term care facilities, assisted living communities, or home & community care environments. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Critical care experience preferred; familiarity with ICU procedures and Level I trauma center protocols is advantageous. Knowledge of specialized areas such as Alzheimer's care, memory care, hospice care, pediatric nursing (including neonatal ventilator management), or behavioral health is desirable. Skills in IV insertion, phlebotomy, specimen collection/processing, and sterile technique are essential. Strong understanding of medical terminology, anatomy & physiology knowledge, and diagnostic evaluation processes. Ability to work effectively with individuals of varying ages and disabilities while demonstrating compassion and professionalism. Excellent communication skills for patient education and interdisciplinary collaboration. Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are a plus.
