Contract Job Duties:

Provide direct patient care to residents in a Skilled Nursing / Long-Term Care facility Administer medications and treatments as prescribed by physicians Monitor and document patient conditions, vital signs, and progress notes Assist RNs and healthcare teams in developing and implementing care plans Respond to patient needs and emergency situations promptly Maintain accurate charting and documentation using PointClickCare or EMR systems Ensure compliance with facility policies, infection control, and safety standards Support residents with daily living activities and compassionate patient care

Job Requirements:

Active LPN/LVN License (Minnesota or Compact State License preferred) Minimum 2 years of Skilled Nursing / LTC experience Current BLS Certification Experience working in Long-Term Care or Skilled Nursing facilities Strong communication and teamwork skills Experience with EMR/documentation systems, preferably PointClickCare Patient care and medication administration experience Documentation and charting experience Ability to work independently in a fast-paced environment

$40.00 - $44.00 per hour
