Job Description

Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, skilled nursing services across various settings, including long-term care facilities, hospitals, outpatient clinics, and community health environments. Your energetic approach and dedication will help foster a supportive atmosphere for patients of all ages, from neonatal to geriatric populations. Responsibilities Deliver high-quality patient care by performing physical examinations, vital sign assessments, and comprehensive patient evaluations. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with medical procedures such as catheterization, suturing, specimen collection/processing, and sterile technique applications. Utilize electronic health record (EHR) systems like Epic, Cerner, or Athenahealth to accurately document patient information and medical records. Conduct patient education on health coaching topics including medication management, nutrition, dementia care, Alzheimer's care, and chronic disease management. Collaborate with multidisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure seamless transition of care across settings. Monitor patients' physiological responses during procedures such as cardiac catheterization or spinal taps; manage airway and ventilator support when necessary. Perform specialized nursing tasks including IV insertion, venipuncture, tube feeding administration, and catheter care while adhering to infection control standards. Provide compassionate care for individuals with developmental disabilities or behavioral health needs in assisted living or home & community care environments. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation in accordance with NCQA standards and CPT coding practices. Experience Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience in diverse clinical settings such as hospitals (including ICU or Level I/II trauma centers), nursing homes, hospice or palliative care environments. Critical care experience including ICU or emergency room exposure is highly desirable; familiarity with Level I trauma center protocols preferred. Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks to ensure accurate documentation. Knowledge of
ICD-9/10
coding for medical records and diagnostic evaluation processes. Experience working with specialized populations such as pediatrics (including neonatal ventilator management), geriatrics (memory care/dementia), or individuals with disabilities is advantageous. Skills in patient assessment, triage procedures, pain management techniques—including acute pain management—and infection control practices are essential. Demonstrated ability to work effectively in fast-paced environments like urgent care or outpatient clinics while maintaining attention to detail and patient safety standards.
