Licensed Practical Nurse

Concordia is seeking compassionate and dedicated Licensed Practical Nurse to provide high-quality nursing care and support to our residents. This role is responsible for administering medications, coordinating resident care, supervising nursing assistants, and ensuring residents receive exceptional care in a supportive environment.

Current openings:

We are hiring a casual LPN. This would be a minimum of 4 shifts per month, 2 of which would need to be worked on the weekend.

Responsibilities:

Administer medications in accordance with physician orders, policies, and regulations Complete accurate and timely resident assessments and documentation Monitor resident conditions and communicate significant changes to physicians and families Carry out physician orders, referrals, and treatment plans Provide treatments and nursing procedures as needed Supervise and support Nurse Aides by monitoring care quality, documentation, staffing coverage, and daily assignments Participate in shift report communication to ensure continuity of care Assist with direct resident care as needed Maintain compliance with HIPAA, safety standards, and state and federal regulations

Qualifications:

Current Pennsylvania LPN license required; Or multi-state license Compassionate, caring, and resident-focused approach Strong leadership and communication skills Why work for us?
