36/48 Hours per Week Job Summary A compassionate and reliable Licensed Practical Nurse (LPN) is needed to provide high-quality care in a long-term care/skilled nursing setting. This role involves supporting patient assessments, administering medications, monitoring resident health, and assisting in the implementation of individualized care plans to ensure comfort and safety. Key Responsibilities Provide direct resident care in a long-term care environment Assist with patient assessments and monitor health status Administer medications and IV therapy as prescribed Maintain accurate and timely clinical documentation using EMR systems (Matrix) Support and implement individualized care plans Monitor and report changes in patient condition to the care team Ensure adherence to patient safety, infection control, and facility protocols Collaborate with interdisciplinary teams to deliver quality care Required Qualifications Active North Dakota LPN License IV Certification Basic Life Support (BLS) Certification Ability to work rotating night shifts and weekends Preferred Qualifications Experience in Long-Term Care (LTC) or Skilled Nursing facilities Familiarity with Matrix EMR system Core Skills & Competencies Strong clinical and patient care skills Effective communication and teamwork Medication administration and monitoring Attention to detail and accurate documentation Ability to manage workload in a high patient ratio environment (1:24) For more details reach at vgill@navitassols.com or Call / Text at 516-862-1203 .
