Job Description
The Medical Practice LPN provides direct and indirect nursing care to patients under the direction and supervision of a Registered Nurse and/or physician/physician extender. This role delivers safe, efficient, and compassionate care in outpatient practice settings, supporting patient treatment, education, and continuity of care while maintaining compliance with hospital policies and professional nursing standards.
Major Tasks, Duties, and Responsibilities:
Patient Care & Clinical Support Monitor, document, and report signs, symptoms, patient responses, and condition changes to providers. Administer medications under provider direction; process prescriptions via telephone, fax, or e-prescribe. Assist providers with tests, procedures, and specialty-specific interventions. Collect specimens for lab analysis, perform waived testing per JMH protocol, and conduct venipuncture after completion of outpatient lab orientation. Assist with emergency measures such as cardiac arrest; maintain current CPR/BLS certification. Practice Operations & Teamwork Participate in daily team huddles, staff meetings, and quality improvement initiatives. Support orientation of new personnel and assist team members with patient care. Ensure practice operations align with policies, procedures, and Patient-Centered Medical Home standards. May be assigned to other practices or work extended/flexible hours based on staffing needs. Patient Education & Communication Establish and maintain communication with patients and families. Provide education on treatments, medications, and follow-up care. Obtain prior authorizations and schedule referrals or diagnostic testing as ordered by providers. Safety, Infection Control & Compliance Adhere to infection control principles, universal precautions, and hospital safety standards. Ensure proper use of equipment and safe work practices. Participate in the economical use of supplies; stock and maintain clinical areas. Follow hospital and departmental policies, procedures, and regulatory standards. Professional Growth & Development Maintain competency through continuing education, in-service programs, and mandatory training. Demonstrate professionalism, customer service, and ethical conduct in all interactions. Support hospital initiatives and community engagement activities as assigned. Perform other duties as assigned within scope of licensure and hospital policy.Education:
Graduate of an accredited School of Practical Nursing. Current New York State licensure and registration as an LPN required. Current CPR/BLS certification required. If assigned to a URMC specialty clinic, additional EPIC training is required.Experience:
One year of acute care experience and/or one year of physician office experience required.Contacts:
Frequent contact with patients, families, providers, and staff across the practice and hospital.Physical Demands:
Stands and walks most of the day.Working Conditions:
