LPN Visit Nurse Responsibilities:

Complete initial and ongoing assessments of clients' health status and vital signs Implement individualized care plans in coordination with the RN Case Manager and care team Administer medications and treatments per provider orders Monitor condition changes and promptly report updates to the interdisciplinary team Educate clients and families on disease management, safety, and home care best practices Assist with ADLs and promote comfort, safety, and independence Document all care accurately and thoroughly Communicate effectively with providers, caregivers, and team members to ensure exceptional outcomes Licensed Practical Nurse -

LPN Visit Nurse Qualifications:

Active LPN license in Minnesota Minimum 1 year of home care or related clinical experience Strong assessment and critical thinking skills Excellent communication and relationship-building abilities Ability to work independently with confidence Reliable transportation and willingness to travel to patient homes
