Nights (5:30 PM - 6:00 AM)Schedule:
12-hour shifts | Every Other Weekend (EOW) Position Overview "Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a Long-Term Care facility in Platte, SD. The ideal candidate will have strong geriatric nursing experience and be comfortable working night shifts in a skilled nursing environment, delivering high-quality, patient-centered care. Key Responsibilities Provide direct nursing care to long-term care residents in accordance with individualized care plans Safely administer medications and treatments as prescribed Monitor, assess, and document patient conditions and changes Perform wound care, vital signs, and routine nursing procedures Assist residents with activities of daily living (ADLs) including hygiene, mobility, and feeding Report significant changes in condition to the RN and care team Maintain accurate and timely documentation in the electronic medical record (EMR) system Follow all infection control and facility safety protocols Support resident comfort, safety, and quality of life during overnight care Collaborate effectively with CNAs and interdisciplinary team members Qualifications Active LPN license (South Dakota or Compact license required/preferred) Graduate of an accredited Practical Nursing program Prior experience in long-term care or skilled nursing preferred Current BLS certification preferred Strong clinical assessment and documentation skills Ability to work night shifts and every-other-weekend rotation Reliable, compassionate, and patient-focused approach to care Strong communication and teamwork abilities For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
