The Social Worker is responsible for assessing each resident at the time of admission and developing a care plan to meet their individual needs. The Social Worker will actively participate in care plan meetings, support the residents in running Resident Council meetings, support completion of Resident Satisfaction Surveys, and help run the semi-annual memorial services. The Social Worker will meet with residents and family members as needed for support and counseling, based on the care plan created and document significant interactions. The Social Worker will advocate on behalf of residents in an effort to meet their needs, but will provide realistic expectations for the home they are living in. Offering appropriate education and support groups to the community as needed, both residents and employees. The ideal candidate will be available on Thursdays for this 8 hour per week position.

Qualifications:

Licensed Social Worker or working towards licensing Social work skills, including conducting assessments, creating care plans, and providing counseling. Strong communication and interpersonal skills for engaging effectively with residents, families, and interdisciplinary teams. Organization and documentation capabilities, including maintaining accurate and confidential records. Knowledge of applicable regulations and standards in a healthcare or residential care setting. Ability to advocate for residents and assist in accessing resources and services. Prior experience in a healthcare, elder care, or residential care environment is highly beneficial.
