Adult Day Care Facility About the Role ATC Healthcare Services is seeking a qualified and compassionate Licensed Social Worker (LSW/CSW) to join our team in an Adult Day Care setting. In this role, you will be responsible for supporting participants' emotional and social well-being, coordinating care services, and working closely with families and interdisciplinary teams to ensure high-quality, person-centered care in compliance with state regulations. Key Responsibilities Conduct comprehensive psychosocial assessments and develop individualized care/service plans. Provide counseling, emotional support, and crisis intervention when required. Collaborate with healthcare providers, caregivers, and community resources to ensure continuity of care. Assist participants and families with accessing social services, benefits, and community-based programs. Maintain accurate, timely, and compliant documentation of all case records. Participate in interdisciplinary team meetings and contribute to care planning. Advocate for participants' rights, dignity, and overall quality of life. Ensure strict adherence to HIPAA guidelines and all applicable state and federal regulations. Qualifications Active New Jersey LSW or CSW license (required). Minimum of 3 years of experience in social work, healthcare, senior care, or community-based services preferred. Strong interpersonal, communication, and organizational skills.
