Job Description

Job Overview We are seeking a dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The LVN will provide high-quality patient care across various settings, including hospitals, outpatient clinics, long-term care facilities, and home health environments. This role requires a compassionate individual with strong clinical skills, capable of working with diverse patient populations such as seniors, pediatrics, and individuals with developmental disabilities. The ideal candidate will be proficient in EMR/EHR systems, possess excellent assessment skills, and demonstrate a commitment to patient safety and comfort.
JOB SUMMARY
The Nurse works closely with the clients and medical staff; aiding in the areas of medication observation, medical appointment scheduling, and overall client health.
QUALIFICATIONS
  • Active RN, LVN, or LPT license.
  • Valid driver's license if required to transport clients.
  • Must have a completed TB test, CPR/First Aid certification, and health screening.
  • Working knowledge of Microsoft Word, Excel, and Outlook.
Must have a valid drivers license
PREFERRED QUALIFICATIONS
  • 2 years of experience in a mental health treatment setting that involves direct client care.
ESSENTIAL DUTIES & RESPONSIBILITIES
  • Responsible for observation, documentation, auditing functions, and record keeping of all observed, inventoried, destroyed, and prescribed medications.
  • Assesses patients with undiagnosed or diagnosed conditions, and assists in ascertaining clinical medication management with Nurse Practitioner, and Medical Director.
  • Assist clients with all self-administered medications via monitoring as directed by the appropriate health care professional (i.e. Medical Director).
  • Monitor client's vital signs for signs/symptoms of medication reaction.
  • Assists in scheduling medical appointments if needed for clients along with client's care team; including medical, dental, psychiatric, etc.
  • Educates and counsels clients and other organizational health care providers on optimal use of medications (i.e. proper use, and avoidance of over medicating).
  • Manages medication observation record sheets and systems.
  • Conducts rounds on clients when necessary.
  • Administer tuberculosis tests on clients upon intake.
Pay:
$30.00 - $34.00 per hour
