Long-Term Care Registered Nurse

Job Overview:

We are seeking a dedicated and compassionate Long-Term Care Registered Nurse (LTC RN) to provide high-quality nursing care to residents in a long-term care setting. This weekend-focused role is ideal for a skilled RN who thrives in a collaborative environment and is committed to supporting the health, comfort, and dignity of elderly and chronically ill patients.

Key Responsibilities:

Deliver direct nursing care to long-term care residents in accordance with care plans. Perform comprehensive resident assessments and ongoing evaluations. Administer medications and treatments as prescribed. Monitor and document patient conditions, changes, and responses to care. Collaborate with physicians, nurse practitioners, and interdisciplinary care teams. Develop, update, and implement individualized care plans. Provide wound care, pain management, and chronic disease management. Respond to medical emergencies and initiate appropriate interventions. Educate residents and families regarding health conditions and care plans. Ensure compliance with facility policies, state regulations, and nursing standards.

Qualifications:

Active Registered Nurse (RN) license in the state of practice or compact licensure. Current BLS Certification (AHA preferred). Minimum of 2+ years of nursing experience, preferably in long-term care, skilled nursing, or geriatric settings. Strong clinical assessment and critical thinking skills. Ability to work independently during weekend shifts. Excellent communication and documentation abilities. Preferred Qualifications Experience with electronic medical records (EMR) systems. Prior experience in skilled nursing facility (SNF) or long-term care environment. Strong background in dementia, chronic illness, or palliative care. IV therapy and wound care experience.

6:00pm - 6:00am (Friday - Sunday) (36 Hrs. per Week)
