Job Description
About McIntosh Trail At McIntosh Trail Community Service Board, we offer individuals experiencing symptoms associated with mental illness, addictive disease and/or developmental disability the hope for optimal functioning and recovery by providing quality behavioral health services and supports. Summary Under general supervision, provides general direct nursing care to mental illness, addictive diseases, intellectual developmental diseases, or disabled individuals. Care includes providing medical treatments, administering medication, and giving injections. Responsibilities
- Responsible for performing assessments on the individuals daily and as needed related to changes in behavior/symptoms and notifying appropriate staff of changes in behavior/symptoms.
- Responsible for completing required documentation for services rendered by utilizing the recognizable and acceptable documentation formats for nursing assessments, ensuring that documentation reflects the individual's treatment goals and objectives.
- Responsible for performing a chart check/chart review throughout the shift; ensuring that Provider's orders are accurately transcribed on the E.M.A.R. and My Avatar.
- Responsible for ensuring proper documentation on the E.M.A.R. when administering medications per policy, while ensuring that the eight rights are being performed.
- Responsible for ensuring that all medication errors are well documented and passed on the RN.
- Responsible for relaying needed information to psychiatric staff, reporting sudden/new changes in individual's behavior to appropriate psychiatric staff; communicate pertinent information to the RN for continuity of care.
- Responsible for supervising Mental Health Techs and ensuring that they are monitoring and interacting with the individuals in an appropriate manner.
- Responsible for collecting blood and other laboratory specimens and waived testing, ensuring that lab logs are maintained and that lab draws/waived testing/results are documented correctly in the EHR.
- Responsible for assisting RN in CSC with assessing and triaging individuals presenting to the Crisis Service Center when Navigation staff is unavailable or as needed.
- Other assigned duties as directed.
- Maintain compliance with all company policies and procedures. Minimum Qualifications
- Vocational / Technical degree from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse AND six months of nursing experience.

Some positions require a valid Georgia's Driver's License. Physical Demands may include- Ability to lift and carry up to 25/50 lbs.
- Sitting, standing and/or walking, pushing/pulling, handling, reaching, bending, twisting, crawling, kneeling, squatting, climbing (ladders or stairs), overhead reaching.
