Job Description

Assistant Director of Nursing (LPN) The Shores of Worthington Are you a strong nurse leader with a passion for both clinical excellence and regulatory compliance? We are seeking a dedicated Assistant Director of Nursing to join our leadership team. This unique role combines clinical leadership with oversight of the assessment and care planning process to ensure the highest quality of care for our residents.
Key Responsibilities:
Assist the Director of Nursing in managing the nursing department, ensuring high standards of care and compliance with state/federal regulations. Oversee daily clinical operations, staff supervision, scheduling, and support for the nursing team. Lead and participate in quality improvement initiatives and regulatory surveys. Coordinate and oversee the Minimum Data Set (MDS) assessment process, ensuring accuracy and timeliness. Work collaboratively with interdisciplinary team members to develop individualized resident care plans. Monitor documentation for accuracy and compliance with care plans, regulatory requirements, and facility policies. Educate and mentor nursing staff in MDS processes, care planning, and clinical best practices.
Qualifications:
Current LPN license in Minnesota. Minimum of 2 years of nursing leadership experience in a long-term care or skilled nursing setting. Strong knowledge of MDS 3.0, care planning, and RAI guidelines. Excellent organizational, communication, and leadership skills. Ability to balance clinical oversight with administrative responsibilities.
