Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval. Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care. Discharges patients in inpatient facility at end of episode (EOE) Obtains patient-related attachment renewal. Participate in High-Risk Meetings and ensure that the best provision of services is offered. Available to provide direct care, as needed. Answers telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties. Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested), providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns. What is Required? Graduate of an accredited school of professional nursing Current Certified Licensed Vocational Nurse or Licensed Practical Nurse 1 year of home care or hospice experience Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations Able and willing to travel up to 50% as needed to support business and patient needs Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver's license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager.Equal Employment Opportunity:
