Job Description

SCHEDULE
Full-time; 36 hours per week; Four 9-hour shifts; Rotating weekends and holidays; Some call The LPN Hospice Nurse will provide care to hospice patients in accordance with the care plan established by the interdisciplinary team. A Hospice Licensed Practical. Nurse (LPN) provides skilled, compassionate end-of-life care, focus on pain management, symptom control and emotional support for patients and families under RN supervision. Working in homes or facilities, they manage medications, perform assessment, assist with daily living (ADLs), and provide vital education during terminal care. Graduate from an approved Practical Nursing Program with current Iowa license. Certification in Basic Life Support is required within 3 months of date of hire. Previous nursing experience, often in home health, long-term care, or acute care is preferred. Strong communication, empathy, ability to work independently in home settings, and proficiency in pain management techniques. Provides nursing care to patients within the scope of his/her practice as a licensed LPN. Maintains accurate, timely clinical notes and documentation of patient status and treatments. Provides care with a focus on patient comfort, pain and symptom management. Administers medications, changes dressings, monitors vital signs and manages comfort-focused care according to the care plan. Maintains a cooperative working relationship with other personnel involved in providing care to the hospice house patients. Nurse will assist with the immediate after care needs of the patient and family. Works weekends and holidays on a rotating schedule. Educates caregivers on comfort measures, provides emotional support and assists with care during the final stages of life. Makes clinical decisions independently (and in collaboration with RN) based on scientific principles, systematic approach and experience.

Documentation reflects plan of care, supplies, equipment and services provides. Assures that documentation is complete including vital signs, intake and output, patient care, assessments, treatments, etc. Supervises Home Care Aide (HCA) as needed per CMS Guidelines.
